Requesting Your Medical Records

Your medical records are an important part of your healthcare story and contain valuable details about your care. Specifics about your visits, diagnoses, treatments, medications, and test results are all there to keep you and the providers you designate informed and involved.

Having seamless access to this information helps you stay organized, make decisions, and share accurate details with other healthcare providers when needed.

At PREMIER Orthopaedic & Trauma Specialists, we want every patient to have easy access to their health information, so we make it simple for patients to obtain copies of their records. Most records are available directly through our patient portal, where you can download them for free and skip the processing fees associated with in-office requests.

Why You Might Need Your Medical Records

There are many reasons you may need a copy of your medical records:

  • Continuity of care: Sharing your medical history between healthcare providers helps coordinate care and treatment progress with the most accurate, up-to-date information.
  • Personal reference: Having copies of your test results, surgical reports, or rehabilitation notes allows you to track your recovery and remain an active participant in your care plan.
  • Insurance or legal documentation: Some forms or claims require official medical documentation, and the patient portal allows for quick access to what you need, when you need it.

No matter the reason, we try to make the process as simple and secure as possible.

How to Access Your Records

The easiest and fastest way to obtain your medical records is through our patient portal. Once logged in, you can view and download:

  • Office visit summaries and progress notes
  • Imaging and test results
  • Medication lists and treatment updates
  • Appointment and billing information

These documents can be printed or shared electronically with another provider without any processing fees.

If you need records that are not available in the portal, you can still submit a formal medical records request through our office. Mailed paper copies may include a small administrative fee to cover processing and postage.

Let us know if you need assistance identifying exactly which records you need or if you have any questions about the request process.

Protecting Your Privacy

Your medical information is protected by strict federal privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA). Access through the patient portal is fully encrypted so that your information stays secure and confidential.

Only you (or an authorized representative with your written consent) may request your medical records. For your protection, the practice verifies all requests before releasing any information.
